Adult Plan

  • 2 exams
  • 2 cleanings
  • 2 fluoride treatments
  • 1 set of recommended x-rays
  • 1 emergency visit, with necessary x-rays included

Child Plan

  • 2 exams
  • 2 cleanings
  • 2 fluoride treatments
  • 1 set of recommended x-rays
  • 1 emergency visit, with necessary x-rays

Perio Plan

  • 3 exams per year
  • 3 fluoride treatments
  • 1 complete set of recommended x-rays
  • 1 emergency visit, with necessary x-rays included

Perio Plan

  • 4 exams per year
  • 4 fluoride treatments
  • 1 complete set of recommended x-rays
  • 1 emergency visit, with necessary x-rays included

Terms and Conditions:

  • This is not dental insurance
  • Cannot be combined with other insurance or senior courtesy
  • Membership will auto-renew each year on the renewal date
  • Membership plan is specific to one person and cannot be transferred
  • No termination fee to cancel; however, no refunds will be issued for the remainder of the year/months membership fees were paid
  • Services provided at this location only
  • Covered services not used within the specified time period will not be carried over or transferred

Details and Limitations:

  • Adult plan is for patients 13 and older.
  • Child plan is for patients 12 and under.
  • If a patient presents with a periodontal infection, a personalized treatment plan will be presented at an additional cost (discounted 10%). The Perio Plans are strictly for preventive maintenance care.
  • Discount is not applicable to dental products sold in the office.
  • Services must be paid in full at time of service.

Disclosures:

Member is required to pay for all services provided by Sunshine Dentists that are not specified in this agreement. This agreement standing alone does not satisfy the health benefit requirements as established in the federal Patient Protection and Affordable Care Act (P.L. 111-148), as amended. Member is encouraged to obtain and maintain insurance for services not provided by this agreement or by Sunshine Dentists. Sunshine Dentists will not bill a health carrier for services covered under this agreement.
